EP09.03. Local Ablation Induces Abscopal Effects and Improves the Efficacy of Continuous ICIs in Advanced NSCLC Patients with Oligo-progression - PDF(Slides)

The study investigated the use of local ablative therapy (LAT) in combination with immune checkpoint inhibitors (ICIs) for advanced non-small cell lung cancer (NSCLC) patients who experienced oligo-progression after ICIs treatment. The researchers aimed to determine the incidence of abscopal effect after LAT and its association with treatment efficacy.<br /><br />The study included NSCLC patients with oligo-progression after ICIs treatment. LAT consisted of stereotactic body radiation therapy (SBRT), radiofrequency ablation (RFA), and microwave ablation (MWA). Patients were divided into LAT and non-LAT groups using a nomogram.<br /><br />The results showed that the LAT group had a significant longer time to next treatment (TTNT) and overall survival (OS) compared to the non-LAT group. Of the patients in the LAT group with evaluable distant lesions, 22 showed over 10% tumor shrinkage. The most common severe adverse events were hematologic toxicities, and no unexpected treatment-related toxicities were observed.<br /><br />The study concludes that adding LAT to ICIs significantly improved abscopal responses and outcomes in NSCLC patients with oligo-progression. These findings suggest that local ablative therapy can induce abscopal effects and enhance the efficacy of immune checkpoint inhibitors in this patient population. The combination treatment was well-tolerated with manageable toxicities.<br /><br />This study highlights the potential of using LAT in combination with ICIs to overcome acquired resistance to ICIs in advanced NSCLC. Further research and clinical trials are needed to validate these findings and optimize the use of LAT in this patient population.
