This study examines the survival rates of non-small cell lung cancer (NSCLC) patients with and without brain metastasis (BM). Historically, BM led to poor prognosis and exclusion from clinical trials, but advancements in therapy have improved survival. The analysis included 360 stage IV NSCLC patients, with 37.5% having BM. Of these, 81 had BM at diagnosis and 54 were diagnosed later. 75% of patients received brain-directed treatment, with stereotactic radiosurgery being the most common. The causes of death for BM patients varied, with neurological, systemic, both, and unknown factors contributing. Overall survival did not differ significantly between patients with and without BM. However, patients with targeted mutations and receiving tyrosine kinase inhibitors (TKIs) had better median overall survival compared to those without targeted mutations. When comparing patients receiving TKIs or non-targeted mutation treatments, there was no difference in overall survival based on BM status. Approximately 24% of the total cohort had BM at diagnosis, and the percentage of patients developing BM increased over time. However, after three years of follow-up, the percentage stabilized. The findings suggest that NSCLC BM patients have comparable prognosis to those without BM and should be reevaluated for inclusion in clinical trials. BM constitutes a distinct subgroup within the NSCLC patient population.
