In this study, the potential of Insulinoma-Associated Protein 1 (INSM1) as a substitute marker for Neuroendocrine (NE) subtyping in Small Cell Lung Cancer (SCLC) was explored. SCLC is a highly heterogeneous cancer and is divided into NE-high and NE-low groups based on gene expression. However, current NE markers are not very satisfactory. Therefore, the aim of this study was to find novel NE markers that are both close to NE gene subtyping and feasible for clinical use.<br /><br />A total of 247 surgical specimens were collected for this study. The expression of NE genes and INSM1 protein was measured in 48 testing cases. The results showed that NE classification defined by INSM1 protein expression and NE genes defined by mRNA levels had significant consistency. NE groups also showed consistent clinical characteristics and prognostic trends.<br /><br />In the validation exploration, INSM1 expression was positively correlated with three traditional NE markers (CD56, ChrA, Syn). Notably, in triple-negative cases, INSM1 showed a higher sensitivity. Patients with NE-high and NE(INSM1)-high subtypes tended to be in the advanced stage of cancer and had a poorer prognosis compared to NE-low subtypes.<br /><br />The study findings suggest that INSM1 has the potential to be a marker for NE subtyping in SCLC. It shows good correlation with NE genes, clinical characteristics, and prognostic outcomes. Moreover, INSM1 has a higher sensitivity in triple-negative cases and may serve as a useful marker for prognosis and stratification of patients.<br /><br />Overall, the discovery of novel NE markers, such as INSM1, is important for improving the clinical application of NE subtyping in SCLC. This research contributes to narrowing the gap between research and clinical practice, with the aim of improving patient outcomes.
