EP13.02. Minimally Invasive Surgery for Endobronchial Small Cell Lung Carcinoma following Neoadjuvant Chemotherapy - PDF(Abstract)

The first case involved a 60-year-old male with a 4.2 cm central tumor causing complete atelectasis. The second case involved a 67-year-old female with a small tumor close to the origin of the left lower lobe bronchus. Both patients underwent staging tests with negative results for metastatic disease.<br /><br />In the first case, the patient received two cycles of preoperative chemotherapy and underwent a thoracoscopic right upper sleeve lobectomy and mediastinal lymph node dissection. The final histology showed no residual cancer and the patient recovered well. In the second case, the patient also received two cycles of preoperative chemotherapy and had a thoracoscopic left lower lobectomy. The final histology also showed no residual cancer and the patient had an uneventful recovery.<br /><br />The report concludes that limited-stage small cell lung carcinomas with a bronchial component are rare, but an aggressive multimodality approach can be beneficial. Minimally invasive surgery, even in complex cases such as sleeve lobectomy with bronchial anastomosis, can offer advantages in postoperative recovery without compromising oncological principles. The report also highlights the importance of avoiding pneumonectomy when possible to reduce postoperative complications.<br /><br />Overall, this case report demonstrates the effectiveness of minimally invasive surgery following neoadjuvant chemotherapy for endobronchial small cell lung carcinoma.
