The study aimed to evaluate the effect of prophylactic cranial irradiation (PCI) on the prognosis of patients with limited-stage small cell lung cancer (SCLC) in the era of MRI surveillance. The study included limited-stage SCLC patients who achieved complete or partial remission after chemo-radiotherapy (CRT).<br /><br />The study included 309 patients with limited-stage SCLC between June 2002 and January 2017. Of these, 133 patients achieved complete remission after CRT, with 89 of them receiving brain MRI to exclude brain metastasis before PCI. The cumulative brain metastasis rate was found to be 18.3% in patients who received PCI and 37.9% in patients who did not receive PCI. However, there was no significant difference in overall survival rates between the two groups.<br /><br />In patients who achieved partial remission after CRT, 80 out of 88 patients received brain MRI before PCI. The cumulative brain metastasis rate was 10.2% in patients who received PCI and 44.2% in patients who did not receive PCI. The overall survival rates were higher in patients who received PCI compared to those who did not receive PCI.<br /><br />The study concluded that PCI significantly reduced the incidence of brain metastasis in patients with limited-stage SCLC who achieved complete or partial remission after CRT. However, PCI did not have a significant positive impact on overall survival in patients who achieved complete remission. The study suggests the need for further prospective randomized studies to evaluate the role of PCI in these patients.
