EP13.07. Canadian Consensus Recommendations: Use of Radiation Therapy among Patients with ES-SCLC receiving Immunotherapy - PDF(Abstract)

This document presents the Canadian Consensus Recommendations for the use of radiation therapy (RT) among patients with extensive-stage small-cell lung cancer (ES-SCLC) receiving immunotherapy. The goal of the study was to provide guidance on the appropriate use of RT and magnetic resonance imaging (MRI) surveillance in this patient population.<br /><br />A pan-Canadian expert working group consisting of radiation oncologists and medical oncologists addressed eight clinical questions related to RT and MRI surveillance in ES-SCLC patients receiving immunotherapy. A targeted literature review was conducted to identify recent publications in this area. Fifteen recommendations were developed, and online voting was conducted to gauge agreement with each recommendation.<br /><br />Based on the available evidence and clinical experience, the experts recommended that patients with a response to chemo-immunotherapy, good performance status, and limited extra-thoracic disease should be considered for consolidation RT. RT can be initiated after completion of initial chemo-immunotherapy or during maintenance immunotherapy. Brain MRI surveillance should be conducted for all patients who respond to concurrent chemo-immunotherapy to guide decision-making regarding prophylactic cranial irradiation (PCI) versus MRI surveillance alone. MRI surveillance should continue for two years after response to initial therapy. PCI can be considered for patients without central nervous system involvement who have a response to chemo-immunotherapy and good performance status. Concurrent treatment with PCI and immunotherapy is appropriate after completion of initial therapy.<br /><br />These consensus recommendations provide practical guidance on the appropriate use of RT and immunotherapy in ES-SCLC patients while awaiting additional clinical data.
