EP14.01. Cytologic Predictors and Novel Markers of Malignant Mesothelial Proliferation (mesothelioma) in Pleural Effusion - PDF(Abstract)

This presentation discusses the diagnosis of malignant mesothelioma (MM) in pleural effusion using cytology and immunocytochemistry. MM is a type of thoracic tumor that is often diagnosed at a late stage, reducing the chances of successful treatment. Currently, histologic criteria for MM diagnosis require invasion in surrounding fatty tissue, which is not applicable to cytology samples. Pleural effusion, a common manifestation of MM, can be easily accessed for diagnosis compared to a surgical biopsy. However, cytomorphological criteria for diagnosing MM are not well-defined, and molecular studies are expensive and not widely available. <br /><br />The study aims to establish a set of robust and reproducible morphological criteria with the help of immunocytochemistry to confidently diagnose MM using cytology. The researchers will evaluate 37 pleural effusions diagnosed with MM and compare them with 20 cases of benign mesothelial proliferation. Eleven morphological criteria, including chromatin change and presence of abnormal mitoses, will be assessed for each sample. Additionally, an immunohistochemical panel will be performed on 37 pleural biopsies diagnosed with MM and 20 negative cases to develop a diagnostic algorithm.<br /><br />The results of the study are currently being analyzed, with a panel of experts reviewing and scoring the morphological criteria for the selected pleural effusions. The immunocytochemistry panel is being performed on positive and negative pleural biopsies to establish the diagnostic algorithm. Early detection of MM using cytology and molecular approaches has a significant impact on patient survival, and a conclusive positive cytology diagnosis through morphological criteria and immunocytochemistry could help expedite treatment.<br /><br />Keywords: Mesothelioma, cytology, immunophenotyping.
