Pdf Summary
This document discusses the use of robot-assisted thoracoscopic surgery (RATS) for mediastinal tumors at a hospital in Tokyo, Japan. The hospital has been performing three-port VATS surgeries since 1999, with the surgeon always on the right side of the patient and the assistant watching through an inverted monitor. In 2019, RATS was introduced at the hospital, with the same port placement and confronting view maintained. The robotic ports were placed vertically instead of horizontally.

A retrospective analysis of 23 patients who underwent RATS for mediastinal tumor resection between October 2019 and December 2022 was conducted. The mean age of the patients was 63, with a male to female ratio of 9:14. The majority of the tumors were located in the anterior mediastinum, with a mean diameter of 35mm. The surgical procedures performed included total thymectomy, partial thymectomy, and tumorectomy. The mean operation time was 186 minutes, with no conversions to thoracotomy or postoperative complications. The average length of hospitalization was 3 days.

The advantages of RATS with a confronting view include the surgeons' familiarity with the surgical view, thorough observation of intrathoracic structures, and the ability to safely dissect and cut veins. The authors note that future work for RATS includes efficiently controlling minor bleeding and expanding indications to include total thymectomy combined with surrounding tissues.

In conclusion, RATS for mediastinal tumors can be safely accomplished using the same techniques as VATS with the same view. However, more work needs to be done to further improve and expand RATS procedures.
