This study aimed to analyze the significance and risk factors of lymph node metastasis (LNM) in patients with thymic malignancies. The researchers collected data from the SEER database from 1988-2015 (training set) and 2018-2019 (validation set). Logistic regression was used to identify risk factors for LNM.<br /><br />The results showed that LNM was identified in different percentages of patients with different types of thymic tumors. Logistic regression identified several risk factors for LNM, including the T3-4 stage, thymoma B3, thymic carcinoma, thymic neuroendocrine tumor, and M1 stage.<br /><br />Based on these risk factors, a scoring system was developed to predict the risk of LNM. The patients were classified into four groups based on their risk score: low risk, intermediate risk, high risk, and very high risk. The incidence of LNM increased with the risk score.<br /><br />LNM had a significant adverse impact on overall survival (OS) but not on cancer-specific survival (CSS). Postoperative radiotherapy tended to improve OS and CSS for patients with LNM.<br /><br />The study concluded that surgical resection with regional lymph node dissection is recommended for thymic malignancies with a high potential for LNM. The risk score based on local extension, distant stage, and histology type can predict LNM of thymic tumors and guide clinical practice.<br /><br />In summary, this study provides valuable insights into the significance and risk factors of LNM in patients with thymic malignancies. The findings highlight the importance of lymph node sampling or dissection in surgical strategies and suggest a risk score that can help predict LNM and guide treatment decisions for thymic tumor patients.
