LG primarily affects the lungs but can also involve the skin, central nervous system (CNS), and other organs.<br /><br />The grading of LG, which determines the treatment approach, is based on biopsy results and ranges from level 1 to level 3. The authors present a case of pulmonary LG with trigeminal nerve involvement in a 65-year-old man. The patient initially had lung lesions and was diagnosed with grade 3 pulmonary LG. He underwent treatment with 6 cycles of R-CHOP chemotherapy. The patient's LG was monitored using PET/CT and MRI scans, which showed fluctuating presence of tracer-avid pulmonary, cutaneous, and subcutaneous lesions. <br /><br />The patient developed left trigeminal nerve symptoms, and imaging revealed a lesion on the nerve. PET/CT also showed additional lesions in the lungs and skin. Skin biopsy confirmed the recurrence of LG. The patient received systemic therapy and additional treatment for the neurological symptoms. After treatment, imaging showed resolution of the trigeminal lesion but persistent activity of LG elsewhere.<br /><br />The authors emphasize the importance of PET/CT and MRI in staging and monitoring the therapeutic response of LG, especially in lung, CNS, and skin involvement. They highlight that LG can mimic primary or secondary malignancies in the lungs and that careful imaging examination and biopsy are necessary for accurate diagnosis and treatment.
