This study aimed to investigate the role of private health insurance in patients diagnosed with advanced lung cancer in Japan. A survey was conducted at 16 hospitals between 2013 and 2016, collecting data on patients' health insurance, clinical and sociodemographic characteristics, and self-reported outcomes after diagnosis. The study found that over 60% of patients had private health insurance alongside Japan's national health insurance system. The ownership of private insurance was associated with certain sociodemographic factors such as age and employment status. However, there were no significant differences in medical care, financial problems, patient satisfaction, and physical and functional quality of life between patients with and without private insurance, suggesting that the national health insurance system provided necessary financial support to all patients with advanced lung cancer. Additionally, having private insurance was associated with better emotional and social quality of life after diagnosis. This suggests that private health insurance may play a supplementary role in providing medical care for patients with advanced lung cancer in Japan. Overall, the study provides valuable insights into the reality of health insurance in advanced lung cancer patients in Japan and highlights the potential benefits of private insurance in improving certain aspects of quality of life.
