P1.15. Cardiovascular implications of Lung Cancer Screening Program: The Smokers health Multiple Actions (SMAC) study - PDF(Slides)

A study called the Smokers Health Multiple Actions (SMAC) study was conducted to assess the cardiovascular risk profile of individuals enrolled in a lung cancer screening program. The study included subjects aged 55 years or older who were current smokers or had a smoking history of at least 30 pack-years and had quit within the past 15 years, without symptoms of lung cancer. The participants underwent a low-dose computed tomography (LDCT) chest scan for lung cancer screening, and the coronary artery calcium (CAC) score was measured to estimate cardiovascular risk.<br /><br />Out of the 1,112 subjects enrolled, 1,049 had no history of cardiovascular disease and were considered as the cardiovascular primary prevention cohort. The study found that approximately one-third of these subjects were at high or very-high cardiovascular risk. Only a minority of subjects were at low risk, and one-third had no detectable coronary artery calcium. The estimated cardiovascular risk was low in 1.7% of subjects, intermediate in 64.4%, high in 23.4%, and very-high in 10.5%.<br /><br />The study also found that almost half of the subjects in the entire cohort, and more than half of those in the intermediate and higher cardiovascular risk categories, were recommended to initiate statin therapy based on their cardiovascular risk estimation, CAC score, and baseline LDL-cholesterol levels.<br /><br />These findings highlight the importance of implementing cardiovascular primary prevention programs in patients undergoing lung cancer screening to reduce the burden of cardiovascular disease. The study suggests that LDCT chest scanning for lung cancer screening could also provide an opportunity to assess cardiovascular risk and implement appropriate preventive measures for individuals at high risk.
