P1.15. Dedicated Cardiovascular Screening in Lung Cancer Screening: Preliminary Results from the European 4-IN-THE-LUNG-RUN Trial - PDF(Slides)

The European 4-IN-THE-LUNG-RUN (4ITLR) trial aims to evaluate cardiovascular screening within a lung cancer screening program. The trial plans to include 26,000 participants. Current guidelines recommend evaluating coronary calcifications on chest CT scans, but dedicated imaging acquisition and reconstruction are needed for reliable evaluation of Agatston scores.<br /><br />The study included 1,345 participants who met the inclusion criteria of being aged 60-79 years, having a smoking history of 35 pack-years, and being current smokers or having quit smoking within the past 10 years. The participants underwent a single low-dose chest CT scan with high temporal resolution.<br /><br />Based on the CT scan, the researchers evaluated lung nodules and coronary calcifications. They performed automatic evaluation of Agatston scores on dedicated cardiac reconstructions, allowing for reliable evaluation of risk categorization. Of the participants with a positive Agatston score, the median score was 207.1.<br /><br />The results showed that one out of three participants in lung cancer screening was at very high risk for coronary heart disease (CHD). Half of the lung cancer screening population were candidates for preventive treatment for CHD, while 18% were at low risk and did not require preventive CHD medication according to current guidelines.<br /><br />There are some limitations to the study, including the lack of background information on participants' current preventive cardiovascular disease medication or previous diagnosis of cardiovascular disease.<br /><br />Overall, the study highlights the potential for cardiovascular screening within lung cancer screening programs and the need for preventive treatment for CHD in a significant proportion of the screened population. The results emphasize the importance of considering cardiovascular health in lung cancer screening and potentially implementing preventive measures for those at high risk.
