P1.16. Differences in Incidentally Detected Versus Screening Detected Stage I Lung Cancer Surgery Patients - PDF(Slides)

This study aimed to compare the characteristics and outcomes of incidentally detected versus screening detected stage I lung cancer patients. The researchers queried a database at Mount Sinai Hospital for patients who met the inclusion criteria of receiving thoracic surgery for stage IA/B lung cancer and being eligible for LDCT screening. They analyzed patient demographics, comorbidities, diagnosis information, tumor characteristics, procedure details, and post-operative events. The analysis found that incidentally detected and screening detected patients had similar tumor characteristics, surgical approach, and post-operative course. However, incidentally detected patients were significantly older and had more comorbidities compared to screening detected patients. Being female and having more comorbidities were identified as independent predictors of being diagnosed based on incidental findings. Additionally, older age and an increased number of comorbidities were associated with decreased survival. The study also found that only 5% of eligible patients were enrolled in LDCT screening in the US. The authors suggest that incidental imaging can serve as a proxy for LDCT screening in the large population that qualifies but is not participating in lung cancer screening. Overall, this study provides insights into the differences between incidentally detected and screening detected stage I lung cancer patients and highlights the importance of considering incidental findings in the management of lung cancer.
