Lung cancer is a significant cause of cancer-related deaths in Vietnam, and its incidence is expected to rise as the population ages. In an effort to address this issue, a pilot program was conducted to integrate tuberculosis (TB) and lung cancer screening in Vietnam. The program used computer-aided detection (CAD) software to identify individuals who had been screened for TB using chest X-rays, and then referred them for further lung cancer screening using computed tomography (CT) scans.<br /><br />Between October 2022 and February 2023, chest X-ray DICOM files were collected from TB screening events in Ho Chi Minh City and Hai Phong. These files were processed using CAD software to identify individuals with potentially malignant lung nodules. Participants with these nodules were contacted and referred to a Provincial Lung Hospital for a CT scan to assess the risk of malignancy using the Lung-RADS classification system.<br /><br />The results of the pilot program showed that out of 31,297 people initially assessed for TB, 212 (0.7%) were identified with potentially malignant lung nodules through the CAD software. Of those eligible for a CT scan, 59.0% agreed to participate and underwent the scan. In total, 91 CT scans were performed, resulting in the detection of 19 people with malignant nodules with a Lung-RADS score of 4 or 5.<br /><br />This pilot program successfully established a model for early detection of lung cancer through CAD-assisted CT referrals. However, further research is needed to understand the reasons for refusal in order to improve outreach strategies. Additionally, the CAD threshold for identifying potentially malignant nodules on chest X-rays should be assessed to optimize the effectiveness of CT referrals.<br /><br />Overall, this integrated screening program shows promise in detecting lung cancer at an early stage, providing opportunities for timely treatment and improved outcomes.
