P1.17. Program Factors Associated with the Psychosocial Outcomes of Lung Cancer Screening: A Systematic Review - PDF(Abstract)

This document discusses a systematic review that aimed to understand the impact of modifiable service-level factors on the psychosocial outcomes of lung cancer screening (LCS) programs. The review found that the design aspects of LCS programs, such as how to disclose low dose computed tomography (LDCT) results and the use of targeted interventions like smoking cessation interventions, can influence psychosocial outcomes. Multiple studies found that smoking cessation interventions improved readiness or motivation to quit, and shared decision-making and the use of patient decision aids reduced decisional conflict about LCS. However, the impacts of interventions on general psychological outcomes varied. The review also found associations between some service design elements, such as high-value discussion with a clinician and communication following LDCT results, and reduced psychological burden. The LDCT scan experience was non-consequential for most participants in two studies. Additionally, earlier non-LDCT results were found to prime patients' subsequent psychological experiences of LDCT results. The review suggests that LCS program design should focus on minimizing psychosocial harm for participants and future research should test the impact of program design on psychosocial outcomes in real-world settings. Feasible methods of managing the psychosocial experiences of LCS should be developed to complement the implementation requirements of large-scale screening programs.
