This study aimed to identify the barriers to completing low dose CT scans (LDCT) for lung cancer screening. Despite recommendations and insurance coverage, only 5% of eligible individuals in the United States have undergone lung cancer screening. The study focused on patients who had completed the shared decision-making visit but did not complete the LDCT scan. <br /><br />A cross-sectional study was conducted on patients at a tertiary-care, academic hospital who had completed the shared decision-making visit and had a LDCT ordered between July and December 2022. Out of the 380 patients included, 87% completed the LDCT scan while 13% did not. Patients who did not complete the scan were less likely to be married, have emphysema, or have a family history of lung cancer but were more likely to be current smokers. Demographic factors such as age, sex, race, insurance status, and income level were not significantly associated with completing the screening. <br /><br />Phone surveys were conducted with the patients who did not complete the scan. It was found that 89% of respondents understood the risks and benefits of LDCT and 56% were told by healthcare professionals that they were at increased risk for lung cancer. However, only 11% agreed that they were at high risk for developing lung cancer and wanted to schedule the LDCT scan. <br /><br />The study concluded that there are barriers to completing LDCT scans for lung cancer screening, even when patients are identified as eligible and complete the shared decision-making visit. Patient disinterest and refusal due to misperceptions contribute to the loss of follow-up after the LDCT is ordered. The study suggests that further research is needed to develop interventions to increase lung cancer screening uptake on a national level.
