Patients with interstitial lung disease (ILD) are known to have an increased risk of developing lung cancer, often in the peripheral site of the lung near the visceral pleura. This study analyzed data from patients who underwent surgical treatment for lung cancer with ILD at a hospital in Japan. The aim was to investigate the clinical and pathological stages, outcomes, and predictors of survival for these patients.<br /><br />The study found that there is poor conformity between the clinical and pathological stages in cases of lung cancer with ILD, with frequent visceral invasion being detected at higher rates. The recurrence of lung cancer with ILD was the main cause of death, highlighting the importance of oncologic control for survival. The 3-year recurrence-free survival rate of patients with pleural invasion was worse than that of patients without pleural invasion, although the difference was not statistically significant.<br /><br />Preoperative evaluation of pleural invasion in lung cancer patients with ILD is challenging. Lung cancer with ILD has a high rate of pleural invasion, leading to an upstaged pathological stage and a tendency towards a poor prognosis.<br /><br />The study also observed that pathological upstaging was seen in 57.7% of cases, and pleural invasion and malignant pleurisy were present in 61.5% of cases. The recurrence of lung cancer was the main cause of death in 86.2% of cases.<br /><br />Overall, this study provides insights into the challenges and outcomes of surgical treatment for lung cancer with ILD. It highlights the need for better evaluation and management strategies for pleural invasion in these patients to improve prognosis and survival.
