P1.21. PD-L1 Expression in Surgically Resected Lung Pleomorphic Carcinoma - One-centre Study of 107 Patients - PDF(Abstract)

A study was conducted to analyze the expression of PD-L1, a biomarker for immuno-oncology, in 107 patients with surgically resected lung pleomorphic carcinoma (PC). PC is a rare and aggressive form of lung cancer characterized by the presence of sarcomas. The study aimed to determine the incidence, intensity, and prognostic significance of PD-L1 expression in PC patients. PD-L1 was found to be positive in 64% of cases, with a strong reaction observed in approximately 60% of those cases. Stronger PD-L1 expression was associated with extensive necrosis and intense inflammatory infiltration in the tumor. Positive PD-L1 expression was more common in adenocarcinoma than squamous cell carcinoma, and least common in mixed epithelial carcinoma. However, there were no significant differences in PD-L1 expression based on the presence of sarcomatous components or the type of sarcomatous pattern. The study did not find any significant relationships between PD-L1 expression and the presence of blood and lymphatic vessel invasion, pleural infiltration, or other biomarkers. Additionally, PD-L1 expression did not show prognostic significance for overall survival or disease-free survival. However, when combined with the extent of necrosis, positive PD-L1 expression was associated with a higher risk of disease recurrence. Therefore, the study suggests that PD-L1 expression in pulmonary pleomorphic carcinomas is common and strong, but it may not have a direct impact on patient prognosis. However, when combined with necrosis, it may be a useful factor in identifying patients at higher risk of disease recurrence who require close monitoring for disease progression.
