Pdf Summary
This presentation at the WCLC 2023 conference examines the correlation between tertiary lymphoid structures (TLS) and immunotherapy outcomes in non-small cell lung cancer (NSCLC) patients. The researchers used an AI-powered analysis of H&E whole-slide images to assess TLS and determine its relationship with immune phenotype and immunologic signatures. They collected H&E-stained whole-slide images and clinical data from NSCLC patients who received either immune checkpoint inhibitor (ICI) monotherapy or a combination of ICI and chemotherapy. The AI model, called Lunit SCOPE TLS, was used to segment TLS within the tumor microenvironment. The presence of TLS was then analyzed for its correlation with survival outcomes.

The analysis included a total of 85 patients, with 29.4% of samples containing TLS. The presence of TLS was not significantly different based on PD-L1 expression but decreased in patients with EGFR and KRAS mutations. There was no significant difference in pre-treatment neutrophil-lymphocyte ratio or tumor mutational burden between cases with TLS and cases without TLS. However, the presence of TLS was associated with favorable survival outcomes in terms of overall survival (mOS) and progression-free survival (mPFS).

Higher proportions of TLS within the tumor microenvironment did not correlate with improved survival outcomes. The predictive role of TLS on survival outcomes was not affected by PD-L1 tumor proportion score levels.

In conclusion, the researchers suggest that AI-powered TLS analysis can potentially be used as a predictive biomarker for NSCLC patients treated with ICI. This research contributes to the understanding of TLS and its relationship with immunotherapy outcomes, providing valuable insights for personalized treatment approaches in NSCLC.
