Researchers from the National Taiwan University have found that the presence of tertiary lymphoid structures (TLS) in the tumor microenvironment can predict disease-free survival in patients with resected lung adenocarcinoma harboring sensitizing EGFR mutations. EGFR mutant lung cancer is the most common subtype among all lung adenocarcinomas in Asia, and patients with EGFR mutations have a higher rate of recurrence after surgical resection compared to those with wild-type EGFR. Therefore, finding predictors of disease recurrence is crucial to identify high-risk patients who may benefit from more intensive adjuvant treatments and follow-ups. TLS, which are aggregates of immune cells including B cells and T cells, have been linked to survival and treatment responses in various types of cancer. In this study, the researchers analyzed the tumor microenvironment using multiplex immunohistochemistry staining and found that a higher density of TLS was associated with longer disease-free survival in patients with EGFR mutant lung cancer. There was no significant difference in TLS count based on different EGFR mutation types, stages, gender, or PD-L1 expression. The findings suggest that TLS could be used as a biomarker to guide clinicians in tailoring adjuvant treatments after tumor resection. The study was supported by grants from the National Science and Technology Council and the Ministry of Education in Taiwan.
