P1.24. The Association of Acinar Pattern with Clinicopathological, Molecular Features, and Prognosis in Lung Adenocarcinoma Patients - PDF(Abstract)

This study investigated the association between different levels of acinar pattern in lung adenocarcinoma (LUAD) and clinicopathological characteristics, molecular features, and clinical outcomes. The samples were obtained from surgically resected LUAD patients and divided into three groups based on the percentage of acinar pattern present. Targeted sequencing was performed to detect mutations/rearrangements in known clinically actionable genes. The recurrence-free survival (RFS) and overall survival (OS) were analyzed using the Kaplan-Meier method.<br /><br />The results showed that patients with a higher percentage (at least 50%) of acinar pattern had worse OS compared to patients with lower percentages (5-45%). This association was observed regardless of disease stage. Patients with at least 5% acinar pattern had worse OS compared to those with less than 5% acinar pattern, both in stage I and stage II-IV.<br /><br />The most frequent driver mutation detected in this cohort was KRAS, with the G12C sub-mutation being the most common. In patients with KRAS mutation, both the acinar predominant (AP) and acinar component (AC) groups had worse OS compared to the acinar negative (AN) group. However, there was no significant difference in OS among AP, AC, and AN groups in patients with mutations/rearrangements in EGFR, BRAF, MET, ROS1, RET, and ALK genes.<br /><br />Multivariate Cox regression analysis confirmed that the presence of at least 5% acinar pattern was a predictor for worse OS independent of gene mutational status, staging, and other histological subtypes of LUAD.<br /><br />In conclusion, this study demonstrates that LUAD patients with a higher percentage of acinar pattern have significantly worse OS compared to those with lower percentages. Additionally, having at least 5% acinar pattern is an independent indicator of poor prognosis in LUAD.
