P1.25. Prognostic Impact of Exon20 Insertion in Surgically Resected Lung Adenocarcinoma Harboring Atypical EGFR Single Mutation - PDF(Slides)

This study aimed to investigate the survival outcomes of patients with lung adenocarcinoma harboring exon20 insertion mutation compared to those with major atypical mutation in the EGFR gene. The study included 843 patients, with 340 patients having exon20 insertion mutation and 503 patients having major atypical mutation. The researchers compared the disease-free survival (DFS), overall survival (OS), and post-recurrence survival (PRS) between the two groups.<br /><br />The results showed that patients with exon20 insertion mutation had a favorable DFS compared to those with major atypical mutation. The adjusted hazard ratio (aHR) for DFS was 0.61, indicating a lower risk of disease recurrence in the exon20 insertion group. However, there was no benefit of adjuvant chemotherapy (ACT) for DFS, regardless of mutation types and tumor stages.<br /><br />On the other hand, patients with exon20 insertion mutation had a worse OS compared to those with major atypical mutation. The aHR for OS was 1.90, suggesting a higher risk of mortality in the exon20 insertion group. Similarly, the PRS was also worse in the exon20 insertion group, with an aHR of 2.50. These findings indicate that patients with exon20 insertion mutation have a poorer prognosis in terms of overall survival and post-recurrence survival.<br /><br />In conclusion, this study reveals that exon20 insertion mutation in lung adenocarcinoma harboring EGFR atypical single mutation is associated with favorable disease-free survival but worse overall survival and post-recurrence survival compared to major atypical mutation. Furthermore, neither exon20 insertion nor major atypical mutation could benefit from adjuvant chemotherapy.
