P1.25. Trends and Disparities in Curative-Intent Treatment for Early-Stage Non-Small Cell Lung Cancer: A Population-Based Analysis of Surgery and SBRT - PDF(Abstract)

This study examines trends and disparities in curative-intent treatment for early-stage non-small cell lung cancer (NSCLC) using population-based data from the Florida cancer registry. The study focuses on the adoption of stereotactic body radiotherapy (SBRT) as an alternative treatment and its impact on racial/ethnic disparities. The data includes information on patient demographics, comorbidities, and treatment.<br /><br />The results show that among the 63,872 early-stage NSCLC patients analyzed, 72.2% received curative-intent treatment, which includes surgery and/or SBRT. The percentage of Hispanic and White patients receiving curative-intent treatment was higher than Black patients, with 73.7% for Hispanics, 73.0% for Whites, and only 60.8% for Blacks.<br /><br />The study also found that the use of SBRT increased annually by 8.1% from 2007 to 2017, while the use of curative-intent surgery declined during 2014-2017. After adjusting for sociodemographic and clinical factors, the odds of Black patients receiving curative-intent surgery were 29% lower than White patients, while Hispanic patients had 17% higher odds.<br /><br />Despite the availability of SBRT, racial disparities in receiving curative-intent treatment for early-stage NSCLC persist. Additionally, approximately 28% of patients across all races do not undergo curative-intent treatment, suggesting that the full potential of SBRT remains unachieved.<br /><br />The study suggests that further research is needed to understand the reasons behind these disparities, particularly focusing on treatment decision-making, patient-provider communication, and treatment compliance/refusal among Black patients with early-stage NSCLC. The findings highlight the need for interventions that address these disparities and ensure equitable access to curative-intent treatment for all patients.
