P1.26. Sublobar Resection is Equivalent to Lobectomy for Stage IA Adenocarcinoma Presenting as Subsolid Nodule - PDF(Abstract)

The objective of this study was to compare the oncological outcomes of patients with stage IA adenocarcinoma presenting as sub-solid nodules who underwent different surgical resections. A total of 624 patients were divided into two groups: one group underwent lobectomy and the other group underwent sub-lobar resection. The sub-lobar resection group consisted of 61% wedge resections and 39% segmentectomies. The primary outcome of interest was lung cancer-specific survival (LCS), with secondary outcomes including disease-free survival (DFS) and overall survival (OS).<br /><br />At a median follow-up of 57.65 months, 16 patients (2.56%) experienced recurrence and 2 (0.3%) died from lung cancer. LCS in the lobectomy group was 100% and 98.2% in the sub-lobar resection group. DFS was 67.4% in the lobectomy group and 71.1% in the sub-lobar resection group, while OS was 68.2% in the lobectomy group and 72.3% in the sub-lobar resection group. There were no significant differences in LCS, DFS, or OS when comparing lobectomy to either wedge resection or segmentectomy. The size of the solid component in the sub-solid nodules did not significantly impact oncological outcomes.<br /><br />Based on these findings, the authors conclude that there is no significant difference in LCS, DFS, or OS between patients who undergo lobectomy or sub-lobar resection for stage IA adenocarcinoma presenting as sub-solid nodules, regardless of whether a wedge resection or segmentectomy is performed. Therefore, sub-lobar resection is the preferred surgical strategy for this specific group of patients with sub-solid nodules.
