P1.26. Trisegmentectomy is Indicated for Non-Small Cell Lung Cancer in the Left Upper Division - PDF(Abstract)

This study aimed to compare the safety and effectiveness of left upper trisegmentectomy (LU3S) and left upper lobectomy (LUL) for the treatment of non-small cell lung cancer (NSCLC) located in the left upper division. The researchers conducted a retrospective analysis of patients who underwent either LU3S or LUL at a single institution between 2006 and 2020. After matching for clinical characteristics, they compared surgical outcomes and long-term oncologic outcomes between the two groups.<br /><br />A total of 251 patients were included in the study, with 147 undergoing LUL and 104 undergoing LU3S. The median tumor size was larger for LUL compared to LU3S. After matching, 86 patients were included in each group, and the median tumor size was similar between the matched groups.<br /><br />The rates of lymphadenectomy and post-operative complications were not significantly different between LU3S and LUL. There was no 30-day mortality in either group. At 6 years, there was no significant difference in overall survival or locoregional-recurrence-free survival between the two procedures.<br /><br />In the subgroup analysis of patients with clinical T1c tumors, the risk of locoregional recurrence and death was not significantly different between LU3S and LUL.<br /><br />Based on these findings, the researchers concluded that LU3S and LUL are equivalent in terms of perioperative morbidity and long-term oncological efficacy for clinically node-negative NSCLC located in the left upper division. This suggests that LU3S may be an appropriate treatment option for T1c tumors, expanding the indications for segmentectomy.
