P1.28. Definition of Resectable Stage III Non-Small Cell Lung Cancer: A Systematic Review from EORTC Lung Cancer Group - PDF(Abstract)

This study conducted by the EORTC Lung Cancer Group aims to provide a definition for resectable stage III non-small cell lung cancer (NSCLC). The therapeutic approach for stage III NSCLC is shifting towards neoadjuvant treatment, with surgery playing an important role. However, the criteria for determining resectability are not well-defined. <br /><br />To establish a uniform definition for resectable stage III NSCLC, a systematic review of literature published from 2007 to 2023 was conducted. The study aimed to identify the definitions used by investigators, the medical specialties involved in the decision-making process, and the required work-up for the decision.<br /><br />After evaluating 374 articles, 84 were considered eligible. The majority of these studies were retrospective series. Forty-nine articles provided some definition of resectability. Factors that were considered against resection included N3 disease, large mediastinal N2 involvement, infiltration of major thoracic structures, and very large tumors with nodal involvement. Factors that were considered acceptable for resection included single N2 station, small size N2 disease, downstaging, and guarantee for R0. The decision of resectability required a discussion in a multidisciplinary board consisting of medical oncologists, radiation oncologists, and thoracic surgeons. Thoracic surgeons were consulted in all cases. PET/CT was considered standard for non-invasive staging, while brain MRI and pathological mediastinal nodal confirmation were also often requested.<br /><br />This systematic literature review is part of a larger initiative by the EORTC LCG to establish a consensus on the definition of resectability in stage III NSCLC. The review identifies important clinical characteristics and work-up requirements to be considered by the multidisciplinary team when making therapeutic decisions. Further analysis of the data will be conducted in the future.
