P2.02. Pneumonitis Rates Before Versus After Adoption of Immunotherapy Consolidation for Locally Advanced Non-Small Cell Lung Cancer - PDF(Slides)

This study examined the rates of pneumonitis before and after the adoption of immunotherapy consolidation for locally advanced non-small cell lung cancer (LA-NSCLC). The researchers hypothesized that the adoption of immune checkpoint inhibitor (ICI) consolidation would increase rates of symptomatic pneumonitis, supporting the need to further reduce lung radiation therapy (RT) dose.<br /><br />The study was conducted at a single institution and included patients with LA-NSCLC who were treated with concurrent chemoradiation. The primary endpoint was grade 2 pneumonitis, while the secondary endpoint was grade 3 pneumonitis. The analysis used inverse probability of treatment weighting to adjust for imbalances between the pre-ICI era cohort and the ICI era cohort.<br /><br />The results showed that the adoption of ICI consolidation was associated with an increase in grade 2 pneumonitis rates, but not grade 3 pneumonitis rates. The study also identified lung radiation dose parameters, such as lung V20Gy and mean lung dose (MLD), that were associated with an increased risk of grade 2 pneumonitis.<br /><br />The findings suggest that stricter lung radiation dose constraints may be warranted with the use of ICI consolidation for LA-NSCLC. The study highlights the importance of optimizing treatment protocols to minimize the risk of pneumonitis in patients receiving immunotherapy. Further research is needed to better understand the factors contributing to pneumonitis and to develop strategies to mitigate its occurrence.<br /><br />In conclusion, the adoption of ICI consolidation for LA-NSCLC was found to be associated with an increase in grade 2 pneumonitis rates. This study suggests that stricter lung radiation dose constraints may be necessary when using ICI consolidation.
