Frailty is a state of increased vulnerability to adverse outcomes and is associated with old age, decreased physiological reserves, and co-morbid conditions. The mFI has been validated as a tool to predict post-operative morbidity and mortality. A score above 0.27 has been shown to predict poor outcomes. <br /><br />The study collected data on patient demographics, co-morbid conditions, operative details, and post-operative course. Logistic regression analysis was performed to identify factors associated with complications following the surgery. An mFI score of 0.18 was used as the cut-off predictor. <br /><br />The study included 857 patients who underwent transthoracic esophagectomy between 2016 and 2022. The average age of the patients was 54.2 years, and the majority received neoadjuvant chemotherapy. The minimally invasive surgery approach was used in 42.5% of cases. Hypertension and diabetes were the most common co-morbidities. <br /><br />The findings showed that there was a trend of increased complications with higher mFI scores, but this trend was not statistically significant. This may be due to the younger age and fewer cardiac or cerebrovascular co-morbidities among the patients included in the study. It was also noted that the mFI does not consider factors such as pulmonary function, sarcopenia, and nutritional status. Therefore, the study concludes that a single frailty index like the mFI may not be applicable to various populations. <br /><br />In summary, the study suggests that the mFI may not be a reliable predictor of complications after transthoracic esophagectomy. Additional factors need to be considered in assessing the risk of post-operative complications in different patient populations.
