P2.22. NUT Midline Carcinoma (NC) Deciphering the Standard of Care in an Ultra-rare Cancer - PDF(Abstract)

This presentation discusses NUT Midline Carcinoma (NC), an ultra-rare and underdiagnosed malignancy with a poor prognosis. NC is caused by the rearrangement of NUTM1, which leads to changes in cell proliferation and dedifferentiation. Due to its low frequency and heterogeneity, there are few clinical trials and no standard of care for NC. To address this, a retrospective study was conducted on patients with NC treated at two French centers, Institut Gustave Roussy and Hôpital Cochin. The study evaluated the efficacy of different curative and palliative treatment approaches.<br /><br />The primary endpoint of the study was Progression Free Survival (PFS), which was assessed based on the chemotherapy regimen in palliative patients and the treatment approach (surgery, multimodal approach, chemo radiotherapy) in radically treated patients. The secondary endpoint was Overall Survival (OS) for different chemotherapy regimens and treatment approaches.<br /><br />The results of the study showed that 11 patients received palliative treatment as their first line of treatment, while 7 received palliative treatment at relapse. The PFS varied depending on the treatment regimen, with the longest PFS observed in patients receiving carboplatin, paclitaxel, and pembrolizumab. The OS also varied based on the treatment approach, with the longest OS observed in patients receiving radical treatment with surgery, chemotherapy, or radiotherapy.<br /><br />In conclusion, the study suggests that CT-immunotherapy should be further explored as a treatment option for metastatic NC. The study also highlights the need for new treatment targets focused on epigenetics and emphasizes the importance of a multidisciplinary approach for radical treatment strategies.
