P2.31. Disparities in Representation of Older Adults, Women, Racial/Ethnic Minorities in Lung Cancer Clinical Trials: Meta-analysis - PDF(Slides)

A meta-analysis conducted by Mayo Foundation for Medical Education and Research revealed disparities in the representation of older adults, women, and racial and ethnic minorities in lung cancer clinical trials. The study analyzed primary reports of phase II/III lung cancer randomized controlled clinical trials from 1989 to 2020.<br /><br />Among the findings, the study found that older adults were significantly under-represented in clinical trials, with an enrollment-incidence ratio (EIR) of 0.63. Both non-small cell lung cancer (NSCLC) and small cell lung cancer (SCLC) trials showed under-representation of older adults. Women, on the other hand, were slightly over-represented in lung cancer trials, with an EIR of 1.05 overall. However, there were opposing trends in NSCLC and SCLC trials, with women being slightly over-represented in NSCLC trials and slightly under-represented in SCLC trials.<br /><br />In terms of racial and ethnic representation, the study revealed significant disparities. Black participants were significantly under-represented in lung cancer trials, with an EIR of 0.49 overall. White participants, on the other hand, were significantly over-represented, with an EIR of 1.46. Hispanic participants were also significantly underrepresented, with an EIR of 0.21. No significant disparity was observed for Asians/Pacific Islanders in terms of enrollment in trials.<br /><br />The study highlights the important issue of under-representation of older adults, women, and racial and ethnic minorities in lung cancer clinical trials. These disparities directly impact the generalizability of trial results and the quality of care delivered to underrepresented populations. The findings emphasize the need for improved representation and reporting of enrollment and outcomes for these groups in clinical trials.
